ROKIHub is building a future association dedicated to the universe related to Rokitansky Syndrome,   with the aim of connecting, supporting and guiding families, patients first and foremost, but with a nod to national and international public information.

The first objective that we are actively pursuing is the creation of a database of Specialists of various disciplines who are knowledgeable about the syndrome, of public and/or private practice, distributed throughout the national territory.

The processing of personal data is explained below, but for convenience the key points of the exposition of what data we collect and how we expose it, are reported in the following lines.

A DATABASE will be created, which will generate a list, inserted on the rokihub.it website, on the Specialists page , public and accessible to all, containing the following information:

ACRONYM;
NAME SURNAME;
SPECIALTIES;
type of business: PUBLIC/PRIVATE;
e-mail address;
CITY, PROVINCE, REGION (if specifically indicated,
ADDRESS Public/Private Studio);
PUBLIC STRUCTURE;
PRIVATE STRUCTURE;
DIRECT LINK to the STRUCTURES, whether PUBLIC and/or PRIVATE;
PHOTO (not mandatory).

The collection of this data takes place through GOOGLE FORM, following the specific responsibilities of data processing.

Authorization for the Processing of Personal Data for Disclosure

Introduction:

This notice is provided in compliance with Regulation (EU) 2016/679 (General Data Protection Regulation, GDPR) and Italian data protection laws. The data subject (i.e., the user filling out a questionnaire) is invited to carefully read this information and provide consent for the processing of their personal data for publication on the website rokihub.com before sharing any personal information or completing any electronic form provided.

Data collection is conducted using Google Forms technology. Therefore, cookies, navigation data, and data entered by the data subject are collected and processed by Google, and subsequently by ROKIHub.

Data Protection Officer (DPO):

The data processor is Google Ireland Limited ("Google"), a company established and operating under Irish law (Registration Number: 368047), headquartered at Gordon House, Barrow Street, Dublin 4, Ireland. Google is appointed as the EXTERNAL DATA PROCESSOR for the purposes outlined (cloud data management for Google Forms and associated services) under Articles 4 and 28 of Regulation (EU) 2016/679 and related data protection laws.

Google is responsible for any personal information shared with third parties for external processing on its behalf, in compliance with the Onward Transfer Principle, as described in the section on user information sharing.

ROKIHub ensures the security and confidentiality of collected data, adhering to current regulations and this notice. Providing data is optional.

Purpose of Processing:

The personal data collected will be processed for inclusion on the "Specialists" page of the ROKIHub website, specifically for informational dissemination. The provided data will be public and accessible to anyone visiting the specified page.

Types of Data Collected:

The personal data processed includes, but is not limited to:

  • Name and surname

  • Medical specialty

  • Email

  • Address

  • Public institution details (name, address, website link, contact link)

  • Private practice details (name, address, website link, contact link)

  • Photograph (optional)

Legal Basis for Processing:

The processing of personal data for the above purpose is based on the explicit consent of the data subject under Article 6(1)(a) of the GDPR.

Processing Methods:

Personal data will be processed using IT and/or telematic tools, ensuring the security and confidentiality of the data in compliance with current regulations.
The data voluntarily provided through the questionnaire will be transformed into an email by the external data processor. This email may be stored within the email reception system used by the data controller.
The data subject may exercise their rights under Articles 15-22 of Regulation 2016/679, such as requesting consultation, updates, correction, or deletion of their data, as well as opposing the processing for legitimate reasons, by contacting the data controller at info.rokihub@gmail.com or info@rokihub.it.

Data Retention:

Personal data will be retained only as long as necessary to achieve the purposes for which it was collected and processed.

Rights of the Data Subject:

The data subject has the right to exercise their rights under Articles 15-22 of Regulation 2016/679, including the right to:

  • Access their personal data

  • Rectify or delete their data

  • Restrict processing

  • Data portability

  • Object to processing

The data subject may exercise these rights by sending a written request to info.rokihub@gmail.com or info@rokihub.it.

Withdrawal of Consent:

The data subject has the right to withdraw their consent at any time without affecting the lawfulness of the processing based on consent before its withdrawal.
